To request an award certificate, complete Award Request Form and return to service center.

Awards will be processed within 2-3 weeks from the date received.

[edit] Where tickets are issued

EVA Air ticket locations. Award certificates are automatically sent to member's address on file after 20,000 miles are accumulated.

Not available.

[edit] Transferability of Awards

Awards may be transferred to anyone at time of award issuance by the person whose name appears on the certificate.

[edit] Changing an Award

Flight changes (excluding date changes): US$20 fee.
